Instructions

The nomination forms for Chapter of the Year, New Chapter of the Year and Volunteer of the Year are editable PDF forms. The process to download, complete and submit the forms is as follows:

  • Click in the "2012 Nominations" box on the award homepage, or on the following links, to download the appropriate form:
  • Depending on your browser preferences, the form will either download to your computer or open in your web browser.
    • If the form opens in your browser, follow these instructions: immediately go to “File” and click “Save As.”
    • Save the file to your computer, in a location where you can retrieve it easily. Use a name that identifies your chapter or the volunteer you are nominating.
    • Close your browser.
  • Once the form is saved on your computer, locate the nomination form where you saved it, and open it.
  • Complete the form as best you can. Save and re-open the nomination form as much as you need to until you are satisfied with the entire nomination.
  • Email the completed nomination along with any required additional documents to the appropriate regional staff person listed at the end of the form. The deadline for submission is January 31.
  • Await news from that regional staff person about the success of your particular nomination.
