The form below will submit your nomination for the Indiana Dollars for Scholars Chapter of the Year Award. The winner of this award, as well as the winners of seven other regional awards across the nation, will be entered into the competition for the National Chapter of the Year Award.

Please note: if the chapter you're nominating was founded in 2004 or later, you should nominate them for the New Chapter Of the Year instead!

Criteria for this award are:

Reports submitted by deadline dates
Excellence in board membership
- Exemplary board participation in programs and activities
- Minimum of 6 active board members representative of community
Achievement in fundraising efforts
Excellence in community public relations efforts
Exceptional scholarship awards program
